I have been seeing this problem on IE8 in XP and on Firefox in Linux.
I fill in the required fields (see first screenshot below), click "Apply for licnese" and it takes me back to the start (second screenshot), with all fields emptied.
Is there something wrong with the web script?

Hi Doug,
You were correct, the webscript was indeed buggy. I have corrected this issue - now only the "name" and "country" field is required on the "About the academic organisation" screen and the webpage correctly shows this requirement as well.
